DEALERSHIP INFORMATION

PAYMENT: Payment is due and payable before order is shipped. Check, money order, wire transfer and credit card (Visa or Mastercard) are accepted. Credit cards, if provided when order is placed, will not be charged until order ships.

RETURNS: Dealer/Installer may submit a request for return with an authorized Lumos Solar representative after product delivery has been made. No returns will be accepted without authorization, regardless of reason. Returns are subject to a 15% restocking fee. Dealer/Installer has 30 days from delivery date to submit a request for return. Valid warranty claims are exempt from the 30 day return period.

WARRANTY: It is the dealer’s responsibility to comply with all of the manufacturer’s warranty terms and conditions. Warranty services may be provided by Lumos Solar with the approval of the individual manufacturer.

SHIPPING: Please specify whether shipment as a complete order is acceptable or, if there are time constraints, if partial shipments are acceptable (additional shipping charges will apply). Lumos Solar is not responsible for liability or loss suffered by Dealer/Installer due to delayed delivery of product or for damaged, lost, missing, or misdirected shipment.

OWNERSHIP OF LUMOS SOLAR TRADEMARKS: Dealer/Installer acknowledges that Lumos Solar owns or controls all rights, titles, and interests in and to the names, logos, and trademarks, including the Lumos Solar trademark.

LIMITED RIGHT TO USE LUMOS TRADEMARKS: Dealer/Installer may place Lumos Solar’s Trademarks in promotional
literature on Dealer/Installer’s website solely for the purpose of promoting sale of products with the express written consent of Lumos Solar.

DUTIES OF DEALER/INSTALLER: May not make any representations, warranties, or covenants of any kind with respect
to the Products, except as set forth in the Lumos Solar standard warranty.

If this application is approved, I/we agree to comply fully with all the policies, terms and conditions of Lumos Solar, Inc., now in effect or as hereafter amended. I understand that these terms are subject to change at Lumos Solar’s sole discretion. I/we hereby authorize Lumos Solar to check my/our credit and/or any other information contained herein. I/we hereby certify that I/we have carefully reviewed the information contained in this application and that it is true and correct in all respects.
